The "Disclosure,
Privacy Act, and Paperwork Reduction Act Notice" for both
the IRS Form 1040 and the IRS Form W-4 state: "You
are not required to provide the information requested on a form that is
subject to the Paperwork Reduction Act unless the form displays a valid
OMB control number."
- In other words, you are not required to file a
form with a "bootleg" number.
- The Form 1040 and Form W-4 both have the same
OMB control number.
- The same OMB control number on two different
forms is no more valid than the same license plate numbers are on
two different cars.
